Case 255 - The New HCM Risk Predictors - What Matters and What Doesn't
CMR is central to HCM management. Merely reprinting values does not help. Flag the ones that matter, LGE%, LV mass index, LV end-systolic volume index, end-diastolic wall thickness > 30 mm and apical aneurysm.
This paper is an important read for all those who read cardiac MRIs and interpret HCM scans.

It is the first large prospective HCM registry to combine clinical, CMR, genetic and biomarker data — 2,698 patients, 44 centres, followed for nearly 7 years.
Five main predictors

LGE, % of LV mass — HR 1.86

LV mass index — HR 1.09

LV end-systolic volume index — HR 1.28

History of heart failure — HR 2.89

NT-proBNP — HR 1.41
Of these, the first 3 are determined on CMR.
CMR is centre-stage in the diagnosis, management and follow-up of patients with HCM. We should know what to include and what not to in our reports.
