65-yrs old was diagnosed to have apical HCM without fibrosis in 2017.
8 years later, a repeat MRI shows apical aneurysm and fibrosis due to ischemia.
Ischemia in apical HCM appears to be universal and may be manifest at presentation or on follow-up
