It is rare to find such a long follow-up with cardiac MRI
This 32-years old presented with HCM in 2004. She was on long-term symptomatic treatment and regular echo follow-up. She came for a follow-up MRI in 2022 with a markedly altered appearance - changed morphology and significant fibrosis.
